These Terms govern your use of Vextara, operated by [Legal entity] (“Vextara”, “we”, “us”). By creating an account or using the service, you agree to these Terms and to our Privacy Policy. If you do not agree, do not use Vextara.

You must be 18 or older to create an account. A parent or legal guardian may create and manage an account on behalf of a child and is responsible for that use. You agree to provide accurate information. The service is available only where our network clinicians are licensed to provide care; we will tell you if your location is not supported.

• Vextara provides an ADHD screener — a DSM-5-based questionnaire plus a short objective attention test taken from your own device — and connects you with licensed clinicians who review the results.
• The app itself does not provide a diagnosis. Any diagnosis, clinical conclusion, and treatment are provided by licensed clinicians exercising their own judgment.
• Vextara is not an emergency service and must not be used for urgent or crisis needs.

The screener and objective results are decision-support measurements, not medical advice or a diagnosis. Do not disregard or delay professional advice because of anything in the service. If you or your child are in crisis, call or text 988, or call 911.

Clinical review, any diagnosis, and treatment are delivered by independent licensed clinicians in our network, subject to their professional judgment and applicable law. A licensed clinician typically reviews your completed assessment within 24 hours. Any diagnosis and care plan are provided following a short online (telehealth) session with the clinician, typically within 72 hours. Telehealth availability depends on your location and the clinician’s licensure, and no specific outcome or diagnosis is guaranteed. Ongoing clinical care — for example therapy, medication management, or prescriptions — is a separate service arranged with the treating clinician and may be billed separately (for example, to insurance).

The assessment fee is [$99], described at the point of purchase and processed by our payment provider; we do not store full card numbers. Applicable taxes may be added. Refunds: you may request a full refund of the assessment fee at any time before a licensed clinician has reviewed your results. Once a clinician has completed your review, the assessment has been delivered. Ongoing clinical care arranged with a treating clinician is separate and governed by your relationship with that clinician.

• Keep your login credentials secure and don’t share your account.
• Provide accurate information and use the service only for its intended purpose.
• Don’t misuse, disrupt, reverse-engineer, or attempt to gain unauthorized access to the service.

Vextara and its content, software, and branding are owned by [Legal entity] or its licensors. You receive a limited, non-transferable right to use the service for its intended purpose.

The service is provided “as is” and “as available,” without warranties of any kind to the extent permitted by law. To the maximum extent permitted by law, [Legal entity] is not liable for indirect, incidental, or consequential damages, and our total liability is limited to the amount you paid us in the [12] months before the claim. Nothing here limits liability that cannot be limited by law, or the professional responsibility of a treating clinician for the care they provide.

You may stop using the service and delete your account at any time. We may suspend or terminate access if these Terms are violated or as required by law.

We may update these Terms; material changes will be notified and continued use means acceptance. These Terms are governed by the laws of [governing law], without regard to conflict-of-laws rules.
